ComPack Program - A Case-Managed Package of Support

What is this service?
ComPack is a case-managed package of support for up to six weeks after discharge from a participating hospital. People need to be referred to this service prior to discharge from hospital.

Do I need this service?
The ComPack program is provided to people who need two or more community services for support in order to return home from hospital. Relevant community services include transport, domestic assistance, personal care, home nursing and meals.

Is there a limit to the assistance available?
Yes, the assistance is provided for up to a six week period only after discharge.

Will this service cost me money?
There are no costs associated with receiving this service, but a financial contribution is welcomed.

In what area is this service available?
This service is available from participating hospitals for people living in the Bankstown or Auburn local government areas.
