Community Options Program - Case Management
What is this service?
Case Management is an approach to service delivery which attempts to ensure that clients with complex and/or multiple problems and disabilities receive the services they need in a timely and appropriate fashion. Instead of providing a direct service, it utilises case managers to direct clients to the appropriate service provider. The case manager takes the time to understand all the client's needs and then uses all possible resources to ensure they are met.
Do I need this service?
You may need this service if you:
• require assistance in organising and coordinating the services you currently receive;
• need support in adapting to regularly changing circumstances;
• find accessing services difficult due to your changing needs;
• require a central point of contact for the purposes of communication given your complex support needs; or
• find it difficult to make the best decision regarding services and do not have any informal support to help you.
Is there a limit to the assistance available?
The assistance provided is based on your level of need, so the duration of Case Management may be from a minimum of three months to a maximum of two years. Being eligible for this service, however, does not mean you will automatically receive it as there is a high level of demand.
Will this service cost me money?
A financial contribution is expected for this service. However, you will not be refused this service if you are unable to pay for it.

This service is available for those who live within the Bankstown or Auburn local government areas.
